We are currently accepting a very limited number of new patients. In order to give each member the time, attention, and quality care that Dr. Fischer MD is built on, we carefully manage the size of our patient panel.

If you’re interested in joining, we encourage you to schedule a free Meet & Greet with Dr. Fischer. This allows you to learn more about the practice and express your interest in becoming a member.

Please note there may be a short wait before enrollment is available, but by attending a Meet & Greet now, you’ll be added to our priority list and contacted as soon as a spot opens up.

When you join my practice, you will know right away that this isn’t a typical doctor’s office.

You will have longer visits, better communication, and no long delays in the waiting room. You will be able to text me with questions, and call me for emergencies outside of normal office hours. You will always see me for your visits – never a nurse practitioner or physician assistant – and you will be seen within a day or two for anything urgent. During your visits, we will take a holistic approach to your care that includes both the “traditional” care you expect from a doctor’s office (prescriptions, labs, x-rays) and “alternative” care such as extensive wellness coaching, the judicious use of scientifically proven supplements, advanced diagnostic testing, and much more.

While Direct Primary Care costs you a monthly fee, it leads to a much more engaged primary care experience that can actually save you money in the long run. For example, an actuarial study, found that my type of practice led to fewer specialist referrals, hospital admissions, and ER visits.

My practice leverages a telemedicine app that allows me to be available after hours, especially for urgent concerns. This dedicated tool allows us to securely text, video chat, and send pictures in real time. If we determine that you need to visit the office, I have same day or next day appointments available.

It’s easy – schedule a visit via secure messaging or call and speak to either me or my wife, and we will book you an appointment. Then come into the office to see me at your scheduled time. You also have the option of scheduling a televisit or phone call for matters that don’t require an in person exam. As a member of my practice there are no additional charges to see me – ever.

As a small practice that is beholden to nobody but you, I go out of my way to serve my patients. If you need to see me for something important and I don’t have a time slot that works for you, text me and we’ll figure out if we can handle your issue by phone/video, or else we’ll find a time outside of “normal” office hours to meet in person.

I will let patients know ahead of time when I will be on vacation or out of town, so that you can arrange to see me beforehand if needed. Once I am on vacation, I will still check my text messages and get back to you for anything that can’t wait until my return. If you have an urgent medical need that must be evaluated in person before my return, I will either arrange to have you seen by a trusted colleague or refer you to an urgent care center.

I only take care of patients who are members of my practice.

However, I never want a patient to feel they are not comfortable with my practice, nor that they aren’t getting great value for their monthly fee. There is no penalty for canceling your membership, so if you join and decide it isn’t for you, just let us know and we’ll cancel your membership as of the following month. A doctor-patient relationship has to be a two-way street of mutual trust and respect, and I have no interest in making it difficult for you to leave my practice if you don’t feel completely confident in my care.

Because my main priority is to make sure I’m available to you while maintaining little to no wait, same day or next sick appointments, and non-rushed, 30-60 minute visits, I cap my patient panel at 300 patients (most primary care doctors care for ten times that number, typically 2,000-3,000 patients).

This works exactly the same as in a traditional medical practice. If I order a lab, test, or medication, you can pay for it via insurance, just as you would with any other doctor. That said, for some labs and other services, I’ve partnered with local companies to provide a discount if you pay them in cash. For patients with high deductible insurance plans, this can be a great option to save additional money.

If you are having a true medical emergency, call 911 or go to the nearest emergency room. You can always call me the following morning to notify me and update me on what transpired. If you aren’t sure what to do, please call and discuss with me at any time. I can help you decide whether it’s something we can take care of or if the emergency room is necessary.

While I do not take care of my patients directly in the hospital (primary care doctors typically no longer go to the hospital; the practice of inpatient medicine has become so complicated that patients are cared for by dedicated “hospitalists” who specialize in hospital medicine), I am happy to stay in touch with you and your family during a hospitalization and to assist in any way I can. I will also see you within a few days of your discharge to review all of your hospital records and make sure any loose threads are tied up.

I welcome all patients – no matter what kind of health plan you do or do not have. Everything done within the walls of my practice is covered by your membership fee. If you need to go outside the office – like for labs, blood work, or specialists – you can use your health plan like you normally would.

YES! You still need some type of health plan coverage for catastrophic events, hospitalizations, ER visits, and visits to specialists.. If you are looking for a cost-effective way to obtain catastrophic care (surgery, accidents, major medical emergencies, etc…), we recommend you enroll in a healthshare. This new type of health plan is affordable, provides great service, and is useful to alleviate any emergency medical bills.

My practice is excellent for the Medicare patient who values having greater access to a primary care physician. My Medicare patients pay my practice fees directly, but Medicare coverage remains in place for everything else: specialists, testing performed outside of our office, and hospitalizations. Medicare will also cover lab services I prescribe as these are performed by an independent lab affiliate. I can still refer, write prescriptions, and fully act as your primary care physician.

Some patients utilize their health savings (HSAs), flexible spending (FSAs) or health reimbursement accounts (HRAs) to pay my fees as “qualified medical expenses”. Legal experts have recommended these patients follow the best practice of paying annually or semi-annually. If requested, we can provide you with an invoice and/or submit a notice to your employer if any questions arise about our programs. Please note that we cannot provide tax or legal advice, and you should consult your own tax advisor for guidance.

I never want a patient to feel trapped in my practice if it isn’t working out for whatever reason. If for any reason you wish to cancel your contract, just let us know, and your subscription will end as soon as that month’s billing cycle is over. If you have prepaid for the year, I will gladly refund you the prorated amount without any additional financial penalty. All contracts are automatically renewed unless a 30-day notice is received by us not to renew.
