Our Philosophy
Dr. Fischer, MD combines the personalized touch of an old-fashioned village doctor with cutting-edge medical care, combining the best aspects of ‘old’ and ‘new’ medicine. We are located in the cozy apartment of an old Victorian building in Marion village, just off of I-195, and have exactly two staff members – the doctor, and his wife.
We aim to recreate the feel of an old-time doctor’s office – a calm, intimate, welcoming place where you are treated with kindness and respect from people who actually know who you are and care about you personally.
About Joshua Fischer, MD
I grew up in Brooklyn, NY, graduated with a degree in history from SUNY Binghamton, and then worked for a few years in the IT industry before deciding to pursue a career in medicine. After graduating from the Warren Alpert Medical School of Brown University and completing my residency training (also at Brown), I practiced in a large primary care group for over a decade, where I treated thousands of patients. I am board certified in Internal Medicine and am a Fellow of the American College of Physicians.
In 2024, I left corporate practice and opened Dr. Fischer MD in order to get out of the insurance driven system and be able to practice medicine the way it should be done – in a small intimate setting that places the needs of the patient first, rather than the dictates of the insurers and the regulators. You can read more about the direct primary care model and how it facilitates better care here.
I provide both conventional medical care and evidence-based holistic treatment to help patients truly optimize their health. I am particularly interested in nutrition and prevention, and working with patients to maximize wellness. My goal is to keep you healthy with the absolute minimal amount of medication.
Outside of work I love cooking, hiking, traveling, reading about history, and spending time with my wife Evelyn and our children.

Evelyn Fischer
I hold a bachelor’s degree from Syracuse University, and worked in the corporate world in technology and design for many years, and later for a variety of nonprofit and educational institutions.
As the office coordinator, I help manage the customer service experience of our patients and will often be the person who greets you when you arrive for an appointment.
Dr. Fischer and I have been married for almost twenty years, and live here in Marion with our children.
